When it comes time to draft your clinical notes, using the SOAP note template is recommended to ensure your clinical notes are supporting your dental billing processes. The acronym SOAP is not new in 2023, but is a tried-and-true template to preparing “clean” dental insurance claim submissions – i.e., all required and supporting documentation, narratives, radiographs, etc. are 100% accurate and complete to show treatment was necessary and rendered. In fact, many insurance carriers now require the entire set of chart notes from the date of service to be attached.
