The New Frontier of Dental Revenue: Human Expertise and AI Working Together

Dental practices are at a crossroads. Amid staffing shortages, glacially slow claims reimbursements, and elevated claim scrutiny, they’re curious whether technology could be the way out. Artificial intelligence (AI) is piquing the interest of many doctors, yet most practices are uncertain about how best to apply it to their advantage.

The Moment Dental Practices are Facing

Claim denials and low reimbursements are among the top issues worrying dental practices, with 55.3% reporting insurance challenges and 54.2% reporting staffing challenges as among their top three concerns.1 Another survey found 63% of doctors have a high degree of anxiety about dental insurance reimbursement levels.2

These worries are not unfounded. Insurance reimbursements are not keeping up with the times for a number of reasons. First, payors are rightfully aligned to reduce fraud, waste, and abuse from their coverage pool. Naturally, to reduce costly and time-consuming review processes, they are pursuing AI to instantly spot anomalies such as missing radiographic evidence or documentation inconsistencies.3 Reports have found that some payors are using algorithms to deny claims at a rapid-fire rate.4

The lag time in claims approvals is another issue. News reports found some practices waiting months for reimbursements from MassHealth Dental, among others.5

In a new podcast, Dr. Marko Vujicic, chief economist and vice president of the Health Policy Institute at the American Dental Association, remarked that “we need to innovate and automate to potentially work in technology to assist humans. We’re at a heyday in terms of being able to bring AI technology to dentistry.” 6

I agree wholeheartedly with Dr. Vujicic that the time is right to adopt AI. Amid all the hype about AI, however, is the salient point Dr. Vujicic was making: Technology is going to assist humans, not replace them. With several years of integrating AI into our workflows, I can confidently share that there is a path forward that will support long-term financial stability for practices: bringing together people with technology.

Why Can’t the Revenue Cycle be Fully Automated?

Reading about all the promises AI is delivering, practitioners often ask, why can’t it be used to automate revenue cycles entirely? Those who have charged ahead and used commercially available tools to replace human activities throughout the revenue cycle have learned the hard way that this doesn’t work. There’s no denying that AI can help us work faster and more efficiently, but for these types of tasks, it requires oversight.

In fact, AI will periodically change the language of dental codes, misinterpret them, or rely on outdated information. In some instances, AI “hallucinates”– that is, it makes up information.7 That doesn’t mean we can’t leverage AI. It just has to be used judiciously to manage the revenue cycle.

The good news is that human intervention can optimize the AI. The complexities of dental billing necessitate billing specialist involvement. These professionals know how to make sense of the intricacies of coding nuance, payor variability, and documentation requirements. Automation alone cannot interpret context, intent, or evolving payor behavior.

The Enduring Value of Human Billing Expertise

The U.S. dental payor system is highly complex and tough to navigate. The average practice must contend with claims denial rates around 15%.8 Each time a claim is denied, it requires more administrative time. Denials impact potential working capital, and write-offs impact actual income.

As any veteran billing specialist will tell you, submitting claims right the first time is the best strategy to avoid these headaches. Billing professionals play a key role in maintaining accuracy and consistency. Their individual judgment and proactive problem-solving can help practices submit clean claims.

Not only do billing specialists have familiarity with hundreds of ever-changing codes, but they also get to know the subtle differences in how each payor reviews claims. They gain this knowledge through experience.

Thousands of dental practices rely on eAssist’s dental billing services to manage their revenue cycles, giving us firsthand insight into how to use AI to effectively augment human expertise.

Where Technology Truly Adds Value in Dental Billing

At eAssist, technology drives efficiency and consistency without replacing human judgment. Our dental billing specialists are supported by two resources from Practice Booster: Dental Coding with Confidence, the premier CDT coding guide trusted by practices nationwide for more than 20 years, and Code Advisor, its fully searchable online companion.

Code Advisor gives our specialists instant access to in-depth guidance on CDT codes, including coding tips, narrative guidelines, documentation requirements, and built-in technology designed to catch mistakes before a claim is ever submitted. Together, these tools enable our team to navigate the complexities of claim submission with confidence, helping to reduce the coding errors and documentation gaps that payers are increasingly using as grounds for denial.

The Code Advisor authors share insights with billing specialists on choosing the right codes and submitting the right documentation. With this tool, specialists can augment their existing experience with guidance on codes, dental plan limitations, and narrative recommendations.

In addition, we added another layer of intelligence to support our billing specialists. By combining the shared wisdom of our specialists with Copilot, our AI-powered software solution, claims are automatically reviewed for potential coding and billing errors before submission to insurance carriers. Copilot recognizes patterns across claims and payors, alerting the specialist to missing documentation or inconsistent narratives.

Copilot is built on collective best practices used by our billing professionals, resulting in claims getting paid faster. Based on an extensive data set across more than 100 offices, eAssist has gained visibility into the outcomes of claims sent within and outside of the Copilot system. On average, the time to first payment is over 40% faster when submitted using Copilot.*

The Power of Combining Human Expertise and AI

In an ideal world, AI and human expertise don’t just complement one another. They propel each other forward.

AI provides speed, scale, and pattern recognition, while billing professionals apply judgment and strategy honed over years of experience. Using this hybrid approach improves outcomes, resulting in fewer denials, faster payments, and a more predictable cash flow.

In the use case here at eAssist, Copilot scours tens of thousands of transactions to swiftly identify potential anomalies and alert the billing specialist. The more alerts the specialist sees, the more they improve their ability to foresee potential claims issues.

Likewise, the specialists “teach” the AI. Each time an alert occurs, the billing specialist either accepts what the AI says or overrides it. From there, the AI tool can ingest these acceptances and overrides, adjusting its parameters based on experience. (This process also involves highly skilled humans-in-the-loop to validate these changes.) In effect, specialists learn from AI, and AI learns from the specialists.

Why Trust and Reliability Matter More Than Ever

No discussion of revenue cycles would be complete without addressing the issue of trust. Practices may be attracted by the potential of AI, but if they can’t trust the platform entirely, all is for naught. They need a way to trust in the process and in the accuracy of submissions. We can’t blindly rely on AI to deliver 100% accuracy in today’s day and age. The technology isn’t at that stage yet.

However, bringing AI’s automation and pattern recognition capabilities together with human expertise is a winning combination. This way, practices can trust in the claims submission process, and payors can trust in the accuracy of submissions. Having these consistent, accurate workflows builds confidence and improves financial stability.

The Future of Dental Revenue

The future of dental billing is not exclusively the purview of people or technology, but a thoughtful combination of both. If dental practices are looking to delve into the world of AI, they must evaluate potential partners based on expertise and long-term sustainability rather than their automation capabilities alone. Trust and reliability must go hand in hand if it’s going to support practices in the long run.

The 20,000-Foot View

The human intelligence behind eAssist’s success is a cornerstone of our enterprise, and that’s not going to change. Our experts have years of experience supporting our clients in improving revenue cycles.
